The US has controlled the Bay since before the Cuban revolution, in fact as far back as 1898. It has a perpetual lease of the land, although Cuba retains autonomy.

The present Cuban government isn't thrilled about its presence and views the original treaty as illegal but can't practically do anything about it.

After the Cuban revolution, the situation became much less friendly. At one point, the Cuban government cut off water and electricity. Now the base gets its water from a desalination plant that is very expensive. All supplies are shipped in by barge (or flown in) and the base has to generate its own electricity. Additionally, there is a minefield no-man’s land and guarded fences all the way around the base that are reminiscent of the Berlin Wall. So essentially, Cuba honors a longstanding treaty/lease but that’s about as far as it goes.
